New Digital Forms for Families Available in NYCSA
 

As in previous years, schools must ensure parents complete the Housing Questionnaire, Active-Duty Military Personnel form, and Digital Equity Questionnaire each year. Additionally, note that starting August 15, parents can complete the new digital versions of the following forms in the forms module in the NYC Schools Account portal (NYCSA):

  • Parent-Guardian-Student Ethnic and Race Identification (ATS integration);  
  • Housing Questionnaire (ATS integration); and 
  • Military Opt-Out (ATS integration). 

The below forms were already available digitally in NYCSA:

  • Children of Active-Duty Military Personnel (ATS integration);
  • Digital Equity Family Questionnaire Survey; and
  • ​​​​​​​Media Consent. 

Families who complete these forms will have the results displayed in the Student Profile under “Reports.” Additionally, note that entries in the forms identified with “ATS integration” automatically feed into ATS overnight, as detailed in the Pupil Accounting Forms wiki page. As a reminder, families can also update and verify emergency contact information via the Emergency Contact feature in My Student in NYCSA. 

Schools can still collect a paper version of the forms from families who do not want, or are unable, to complete the forms electronically. The paper version of these forms and other forms, such as the Name and Gender Change Request form, can be found on the Pupil Account Forms wiki or on the Enrollment Forms InfoHub page. The paper versions of the media consent form and the military recruitment opt-out form can be found on Forms and Letters in all DOE Languages Infohub page
